
CAN, Carallel and Isaac Health Partner to Support Families Affected by Dementia Through the GUIDE Program
What is the GUIDE Program?
Caring for a loved one with dementia can be overwhelming. That’s why the Caregiver Action Network, Carallel and Isaac Health have partnered to bring you the GUIDE Program (Guiding an Improved Dementia Experience)—a new Medicare-backed initiative designed to provide essential caregiving support and resources for families navigating the dementia journey. Those eligible will receive:
- 24/7 support with dementia-trained specialists and support staff to answer questions, provide guidance, and offer reassurance whenever you need it
- A dedicated Care Advocate who will guide you through your caregiving journey – they will check in regularly, assist with care planning, and connect you to community support services
- A team of specialized doctors and nurse practitioners to conduct an initial assessment upon enrollment as well as a customized treatment plan tailored to your needs
- Group caregiver training sessions and one-on-one coaching to help you better understand dementia and how to provide the best care for your loved one
- Respite care, allowing a trained home care aide to step in and care for your loved one for up to four hours at a time
- Access to virtual support groups and community connection for families affected by dementia
